In-Person Service Registration

Please complete the form below and select the dates you intend to attend along with the names of additional family or friends who will attend as well.

Temple Israel Covid 19 Protocols:

              • Sign in upon entry to services so that if needed, we can conduct contact tracing.
              • In order to attend, anyone over 5 yrs of age or older must be vaccinated and provide
                proof of vaccination either in advance (preferred) or at time of service.
                Masks must be worn by children under 5 yrs of age.

How to Provide Proof of Vaccination in Advance

              • Text image of vaccination card to 570-690-1452
              • Note: You will only need to provide proof of vaccination once. Anyone who has previously provided your
                Proof of Vaccination to the Temple Office for the High Holidays does not need to provide it again.

Sanctuary will be open on Shabbat mornings beginning Saturday, December 4th at 10:00 a.m. and
for Afternoon Minyan services on Wednesdays and Fridays at 5:15p.m beginning Wednesday, December 8th.
